(1) diabetic foot cold, blood stasis and venous obstruction syndrome
Clinical manifestations: gangrene of limbs, black color, less oozing from the wound, cold limbs, numbness in pain, insensitivity, pale skin, thin and white tongue coating, dark red tongue and deep and thin pulse.
Dialectical analysis: cold disease, blood stasis blocking collaterals, limb ischemia, gangrene, dark purple, cold limbs, numbness in pain, insensitivity, pale skin, thin and white tongue coating, dark red tongue and weak pulse.
⑵ Damp-heat down and blood stasis syndrome
Clinical manifestations: gangrene of limbs, festering and rotten meat, purplish red color, more exudates on sore surface, swelling of limbs, severe pain, redness of skin, Huang Chi in urine, dark red tongue, yellow and greasy fur and rapid pulse.
Dialectical analysis: the pathogenesis of this syndrome is damp-heat downward flow and internal resistance of blood stasis. Due to dampness, blood stasis, vein obstruction, local malnutrition, gangrene of limbs, festering and rotten meat, purplish red wound and much exudate. Damp-heat is pouring down, so the patient's limbs are swollen, the pain is severe, the skin is red, the urine is Huang Chi, the tongue is dark red, the coating is yellow and greasy, and the pulse is rapid.
(3) Syndrome of excessive heat toxin, yin injury and blood stasis.
Clinical symptoms: gangrene of limbs, rotten meat and erosion, burning pain and swelling, purulent exudation of sore surface, obvious purple-black, accompanied by high fever, coma, thirst and polydipsia. The tongue is purple and dark, with Huang Gan coating and rapid pulse.
Dialectical analysis: the pathogenesis of this disease is heat toxin and blood stasis, blood stasis and internal resistance, so the limbs are gangrenous, the pain is intense, the heat is full and the internal decay, so the meat is rotten and erosive, and the heat toxin is blazing, so the thirst is excessive. Burning swelling and pain, purulent exudation on the wound surface. Excessive heat leads to yin injury, so the tongue is purple and dark, the coating is yellow and dry, and the pulse is rapid.
⑷ Deficiency of both qi and yin, essence injury and blood stasis syndrome
Clinical manifestations: limb ulceration, no new flesh, slow healing, dry skin, muscle atrophy, dizziness, fatigue, dry mouth, dark eyes, dark tongue, thin or astringent pulse.
Dialectical analysis: this syndrome is characterized by late gangrene, injury of both qi and yin, essence injury and blood stasis. Due to deficiency of both qi and yin, internal resistance of blood stasis, limb ulceration, great decline of vitality, deficiency of essence and blood, no new blood, short new meat, slow healing, dry skin and muscle atrophy. Deficiency of both qi and blood, vomiting, dizziness, fatigue, dry mouth and dry eyes. Qi deficiency and blood stasis, both qi and yin injury, so the tongue is pale and the pulse is fine or astringent.
