3 Chinese medicine heatstroke heatstroke: 1. Feel the acute syndrome caused by summer evil; 2. Summer wind; 3. Yin and summer heat; 4. People suffering from cold, abdominal pain, vomiting and diarrhea in summer.

3. 1 heatstroke and heatstroke (1, heatstroke in summer [2]), an acute syndrome caused by feeling hot in summer, is the name of the disease [3].

Terminology of Traditional Chinese Medicine (2004): Heatstroke refers to a disease caused by summer evil, which is characterized by sweating due to high fever or dry skin, irritability and thirst, coma and convulsion, or nausea and abdominal pain [1].

Terminology of traditional Chinese medicine (20 10): heatstroke, also known as summer heat, refers to acute and severe diseases characterized by high fever, sweating or dry skin, irritability, thirst, dizziness, convulsions, nausea and abdominal pain [2].

Heatstroke is also called moderate heat [3] and hair [4]. Now it is also called heatstroke, heat spasm, and heatstroke [4]. Chinese medicine believes that summer heat is an acute disease [3]. See Three Causes and One Disease Syndrome, Volume II.

3. 1. 1 The causes of heatstroke are mostly caused by being in the scorching sun for a long time or in the environment of high temperature and heat radiation [4].

3. 1.2 The symptoms of heatstroke are different in clinic [4]. Symptoms include sudden nausea, unconsciousness, body heat, irritability, dyspnea, tight teeth, dry mouth, sweating or no sweat, weak pulse, and even coma and limb convulsions [3].

3. 1.3 The treatment of heatstroke is mild, with only dizziness, fatigue, chest tightness, excessive drinking, body heat, less sweating, nausea and other symptoms. In severe cases, symptoms such as excessive cold sweat on limbs, palpitation, shortness of breath, high fever, coma, convulsions, etc. [4] may occur. When moving the patient to a cool and ventilated place, the method should be to clear away heat and relieve summer heat, and to enlighten the mind. First take Piwendan and Xiaoshu Pill orally, then use Yiyuan Powder, Baihu Decoction and Qingying Decoction [3]. Acupuncture, scraping and ice bath can be used together [3].

Those who are physically weak should give priority to invigorating vitality and less help relieve summer heat [3].

First, the patient should be moved to a ventilated and cool place, and given cold saline, and then treated [4].

① Body acupuncture: For mild cases, Dazhui, Hegu, Neiguan and Zusanli are the main points [4]. Headache plus head dimension; Vomiting plus epilepsy [4]. In severe cases, Baihui, Renzhong, Quchi, Weizhong and Xuan Shi are the main acupoints [4]. The shocking wind adds Yangling Spring and Taichong; Weak limbs with cold veins will moxibustion Shenque and Guanyuan; Comprehensive treatment measures should be taken when the situation is critical [4].

② Scraping method: suitable for mild heatstroke [4]. Gently scrape the back, neck, chest, shoulders, elbows and popliteal fossa with a smooth ceramic spoon dipped in water until the skin turns purple [4].

3.2 Summer wind heatstroke is the name of the disease [3]. Refers to the summer wind [3]. Medical Science Volume 1: "Heatstroke, or Xia Feng, is similar to stroke."

3.3 Yin deficiency heatstroke is the name of heatstroke [3]. Refers to yin and summer heat [3]. "On seasonal diseases and heatstroke": "Jiegu said: If you don't move, you will get heatstroke. Dongyuan said: People who enjoy the cool in summer are called heatstroke. In fact, both of them are evidence of hot summer ... "

3.4 Suffering from cold, abdominal pain, vomiting and diarrhea, heatstroke and heatstroke are the names of the diseases [3]. Refers to those who suffer from cold, abdominal pain, vomiting and diarrhea in summer [3]. The first volume of "Six Causes" said: "Summer and summer are pressing each other, and the desire for food is cold, so I can't help feeling the desire for the room. So I apologized in Shenyang, and I felt cold and wet in summer, so I took advantage of it and suddenly felt stuffy in my chest and abdomen, cold and sweating in my limbs, vomiting and diarrhea. No matter what happens, it is heatstroke. "

4 Western medicine heatstroke often occurs in high temperature and high humidity environment, and it is a disease characterized by disorder of body temperature regulation center, sweat gland failure and excessive loss of water and electrolyte [5]. It is characterized by high fever, dry skin and central nervous system symptoms [5].

4. 1 heatstroke mechanism heatstroke is a serious adverse reaction produced by human body in high temperature environment. Normal human body temperature is managed by the thermoregulatory centers of cerebral cortex, diencephalon, medulla oblongata and hypothalamus. The heat generated by the human body is dissipated through conduction, radiation, convection and evaporation, so as to maintain a proper body temperature. When the outside temperature is too high, long-term sun exposure, damp heat or high temperature environment with no air circulation hinders heat dissipation, heatstroke will occur.

4.2 classification of heatstroke heatstroke is divided into:

4.2. 1 After working in a high temperature environment for a certain period of time, symptoms such as dizziness, headache, thirst, hyperhidrosis, general fatigue, palpitation, inattention and uncoordinated movements appear [5]. The body temperature is normal or slightly elevated [5].

At this time, as long as we improve the environment and have enough rest, the symptoms will soon improve.

4.2.2 Mild heatstroke, in addition to the symptoms of threatened heatstroke, may also cause the body temperature to rise above 38℃, resulting in flushing, sweating, skin burns and other symptoms. Or appear pale, cold limbs, blood pressure drop and other manifestations [5]. There may be early symptoms of respiratory and circulatory failure.

If active and effective treatment is carried out, it often recovers within a few hours [5].

4.2.3 In addition to the above symptoms, severe heatstroke may cause body temperature as high as 40℃, coma, convulsion, respiratory and circulatory failure, and febrile convulsion, leading to hyponatremia, hypochloremia, hypocalcemia and vitamin deficiency.

Severe heatstroke includes heatstroke, heat spasm and heat exhaustion [5].

4.2.3. 1 heatstroke is a fatal emergency. For people who work in a high temperature environment for several hours or for the elderly, frail and chronically ill, it often happens after several days of high temperature. Typical manifestations are high fever (>: 4 1℃), anhidrosis and disturbance of consciousness. The early affected organs are: brain, liver, kidney and heart. Laboratory examination showed elevated white blood cells, abnormal biochemical and liver and kidney functions, and ECG showed arrhythmia and myocardial damage. [5]

4.2.3.2's heat spasm often occurs after intense physical labor in high temperature environment. It is characterized by paroxysmal tonic spasm of limbs, which is most common in gastrocnemius of both lower limbs, often accompanied by muscle pain and abdominal cramps. Laboratory tests showed that the content of sodium and chlorine in blood decreased. [5]

Heat exhaustion in 4.2.3.3 often occurs in the elderly, children, patients with chronic diseases and those who can't adapt to the high temperature climate and environment for the time being. Patients may have hyperhidrosis, fatigue, weakness, headache, dizziness, weak pulse and low blood pressure. [5]

4.3 Treatment of heatstroke 4.3. 1 In general, patients with heatstroke should immediately leave the high-temperature environment, move to a ventilated, cool and dry place to rest quietly, replenish water and salt, and give intravenous drip of normal saline when circulatory failure occurs [5]. Ren Dan, Shidishui and Huoxiang Zhengqi Pill can be taken orally.

4.3.2 Treatment of severe heatstroke Patients with severe heatstroke should take the following measures:

4.3.2. 1 Physical cooling can reduce the body temperature as soon as possible, which can be reduced to below 38℃, and can be cold-bathed. Ice salt water can also be * * [5].

Ice packs can be placed on the head, armpits, groin and other places, and bathing can be wiped with 50% alcohol.

4.3.2.2 medicine took chlorpromazine 25 ~ 50mg for cooling, and added 5% glucose or 0.9% normal saline intravenously 1 ~ 2h [5].

Chlorpromazine, 25 mg perphenazine, 500 ml normal saline, intravenous drip. 65438+ can be used repeatedly after dropping for 0-2 hours and 3 hours. If the effect is not good, you can add 10 mg dexamethasone.

4.3.2.3's symptomatic treatment (1) sedation: diazepam 10mg intramuscular injection [5].

(2) For patients with brain edema and increased intracranial pressure, mannitol can be used for dehydration and 1 ~ 2g/kg infusion for 30 ~ 60min [5].

(3) Correcting the disorder of water and electrolyte and supporting treatment such as acidosis [5]. The daily drinking water needs to reach 3000 ml.

(4) In case of circulatory failure, digitalis should be added as soon as possible.

4.4 To prevent heatstroke, it is important to strengthen publicity and vigorously advocate the policy of putting prevention first to avoid heatstroke.
