Abdominal diseases [Landau Chinese-English Dictionary]
Summarize gastroptosis as the name of the disease. Of the stomach descending to an abnormal position. It belongs to the category of "epigastric pain", "belching" and "miscellaneous addiction" in traditional Chinese medicine. It is mostly due to the weakness of gastric diaphragm ligament, hepatogastric ligament and abdominal muscle, which can not fix the stomach in its original position [1]. Gastroptosis belongs to stomach weakness, which is more common in patients with wasting diseases and weak constitution, and directly affects digestive function. Clinical manifestations include abdominal distension, loss of appetite, stomachache, emaciation, fatigue, belching, nausea, vomiting, bowel sounds, gastroptosis, constipation, diarrhea, shortness of breath, dizziness, palpitation, insomnia, dreaminess, etc.
Gastroptosis refers to a kind of disease whose main signs are that the notch of the small angle of stomach is below the iliac crest line and the duodenal bulb deviates to the left. More common in 30 ~ 50 years old, women are more than men. The exact cause of this disease is unknown, which may be related to factors such as body shape and diet. Modern western medicine has no specific treatment except symptomatic treatment.
Gastroptosis is equivalent to "slow stomach" in traditional Chinese medicine. This name first appeared in Huangdi Neijing. There is a cloud in Huangdi Neijing Soul Pivot: "The spleen should be fleshy ... If the body of the meat is not weighed, it will not be conducive to lowering the tube. If the meat is not firm, the stomach will be slow. " It is clearly pointed out that the position of the stomach with weak muscles is slower than that of the stomach with low body size and weak muscles. "Its human nature is abundant and thin today, with water flowing through the intestine and dripping loudly, which is called phlegm drinking", which is quite similar to the symptoms of this disease. Although gastric bradycardia is not discussed as a special disease in medical books of past dynasties, many diseases and syndromes related to dysfunction of spleen, stomach and viscera mostly involve symptoms of gastroptosis, such as abdominal distension, fullness and belching.
Early reports on the treatment of this disease by modern Chinese medicine mainly focused on acupuncture, qigong and other non-drug therapies. Since the early 1960s, there have been some samples of clinical data on syndrome differentiation and treatment of traditional Chinese medicine, and they are increasing day by day. Generally speaking, the treatment of gastroptosis is mainly Chinese medicine and acupuncture, each with its own characteristics.
Due to the influence of traditional experience, in the 1960s and 1970s, most doctors believed that the disease was caused by weakness of spleen and stomach and stagnation of middle qi, and generally advocated the treatment of reinforcing middle energizer. With the increase of clinical practice, there have been differences. Although the dysfunction of spleen and stomach viscera belongs to deficiency syndrome, it also has qi stagnation, which should belong to deficiency in the middle and stagnation in the middle. In particular, the course of the disease is long, and the shape and position of the stomach itself have changed obviously, and the blood vessels are pulled and compressed, which makes the venous reflux of the stomach wall obstacle. Blood stasis can occur, so there is another side of blood stasis stagnation. With the deepening of understanding, the treatment tends to be diversified and the curative effect has improved.
In the early days of acupuncture, single needle was the main method. Since 1970s, not only various acupoint methods such as awn needle, scalp acupuncture, acupoint injection and ear acupuncture have been used for this disease, but also some effective new acupoints have been found, which were further verified and summarized in 1980s. At present, it is generally recognized that the application of comprehensive methods, especially the acupuncture point penetration and lifting technique with awn needle, is helpful to improve the curative effect. At the same time, in recent years, there are more and more reports on the application of qigong and massage to treat gastroptosis.
Regardless of Chinese medicine or acupuncture, the total effective rate is about 85%, but the cure rate is still about 40%. Therefore, to further improve the therapeutic effect, it may be a better way to combine various therapies, such as acupuncture and medicine, medicine and qigong.
4 Etiology and pathogenesis of gastroptosis According to the opinions of ancient and modern physicians, the etiology of gastroptosis can be divided into two categories: congenital and acquired. Congenital factors are weak endowment and insufficient physique; The day after tomorrow can be summarized as eating disorders, long illness or excessive delivery, and seven emotions are incompatible. Its disease is located in the spleen and stomach, and its main pathogenesis can be summarized as deficiency and blood stasis, with deficiency as the main factor.
Deficiency of Qi (Yang) or congenital endowment leads to spleen deficiency, qi stagnation, poor health, muscle weakness, sagging stomach, or yin deficiency of constitution, excessive drinking of tea hurts strangers, overeating and cold hurts the spleen, leading to the ascending and descending of middle energizer, fluid stagnation, phlegm turbidity, stagnation of qi and blood, and sagging tendons and veins.
Many factors of yin deficiency and body yang deficiency, or prolonged illness and prolificacy, injury of yin and blood, and burning of five qi in fire, lead to insufficient stomach yin, loss of water in stomach meridians, slow down the speed, and cause gastroptosis.
Blood stasis is mostly due to long-term qi deficiency, inability to transport blood, or long-term illness into the collaterals, blood vessels blocked, leading to blood stasis.
Blood stasis is one of the pathogenesis that people pay close attention to in recent years. 67 cases of gastroptosis were systematically investigated, and it was found that 44.8% of them had blood stasis tongue and 35.8% had blood stasis pulse. In addition, fiber gastroscopy also found blood stasis or pale mucosa, blood stasis with venous exposure. Clinically, it is also confirmed that if the routine lifting method is ineffective, the method of promoting blood circulation and removing blood stasis can obviously improve the curative effect.
5 Diagnosis of Gastroptosis Diagnostic points of Gastroptosis:
(1) slender; Abdominal distension and abdominal pain, especially after meals, are relieved by lying down; Loud heating noise, nausea and vomiting; The stool is irregular, sometimes hidden and sometimes irregular [1].
② Physical examination: In the morning, there is vibration sound in the stomach on an empty stomach, and the abdominal aorta can be palpated in the upper abdomen [1].
③ Gastrointestinal barium meal examination: the line of gastric minor curvature is lower than the line of iliac crest. [ 1]
According to most clinical reports in recent years, gastroptosis can be divided into two types.
1. Spleen deficiency and qi stagnation lead to sallow complexion, loss of appetite, bloating after eating, discomfort with warm qi, lethargy, emaciation, shortness of breath and laziness. The tongue is pale with white fur and thin pulse. If there is phlegm, the water passes through the intestine and makes a sound; Deficiency and cold in the evening mouth, dull pain, liking warm pressure, spitting clear saliva; The female has blood stasis in her mouth, purple tongue coating, ecchymosis and petechiae. Such as liver depression, abdominal distension, belching and vomiting.
2. Deficiency of stomach yin, slightly red complexion, bloating or pain, heartburn, dry mouth and throat, thirst, hunger, bitter taste and bad breath, dry stool and Huang Chi in urine. Red tongue with scanty body fluid, or chapped tongue without coating, thin and astringent pulse; Both blood stasis, purple tongue, exposed sublingual collaterals, do not want to drink water; Qi stagnation, abdominal distension, qi deficiency, fatigue, and mental fatigue.
Gastroptosis is complicated and has many complications, so we should pay attention to distinguish it. Blood stasis can be divided into the above two types. It should be noted that gastroptosis is characterized by blood stasis and solid content, and there are often no symptoms of stabbing pain or fixed pain points. In addition, there is a simple abdominal diagnosis method that can be used as a reference for syndrome differentiation.
7. Treatment of gastroptosis 7. 1 Recovery of therapeutic standard: clinical symptoms disappeared, X-ray reexamination, and lower gastric pole returned to normal position.

7.4 Li Sichi's Medical Records and Old Chinese Medicine Experience
Wang XX, female. 1972,10,19. The patient has a history of gastroptosis, and the stool is sparse, so it is necessary to defecate frequently. At first, I had a little bowel movement. When I met menstrual cramps and caught a cold, my bowel movement was smooth. Poor appetite, full stomach, frequent suffocation and uterine prolapse. Weak pulse and pale tongue. This kind of syndrome of qi deficiency in middle energizer should be treated by reinforcing the middle energizer and strengthening the spleen, and supplemented with Buzhong Yiqi Decoction.
Prescription: Radix Codonopsis 18g, Radix Angelicae Sinensis 9g, Radix Astragali 12g, Rhizoma Atractylodis Macrocephalae 9g, Pericarpium Citri Tangerinae 9g, Rhizoma Cimicifugae 3g, Radix Bupleuri 6g, 2 slices of ginger, Fructus Aurantii 9g, Radix Aucklandiae 6g, 3 jujubes and 6g of licorice.
After taking the above three doses, the effect is obvious, the stool is basically normal, and the rest symptoms are relieved. Tell him to take this prescription regularly to consolidate the curative effect.
Press: In this case, the epigastrium is drooping and swollen, the uterus is descending, the pulse is weak, and the tongue is pale, showing the phenomenon of insufficient middle qi. Qi deficiency in the stomach and intestines not only makes the diet difficult to digest, but also causes loose stools, weak promotion and poor defecation. Spleen deficiency and qi stagnation cause belching. Menstruation is coming, the flow of qi and blood in the body is accelerating, I have a cold, and defecation is normal. The prescription of Buzhong Yiqi Decoction aims at tonifying the middle energizer and supplementing qi, and adds Fructus Aurantii and Radix Aucklandiae to regulate qi. Medicine and syndrome correspond, so it is effective.
7.5 Law of Medication We * * * collected 2 1 prescriptions of traditional Chinese medicine for gastroptosis, and made statistics on the medication of the main prescriptions, which are summarized as follows:
Frequency of application (for example) Literature reports (articles) Drugs > 500 ≥ 15 Cimicifuga, Astragalus membranaceus, Codonopsis pilosula and Atractylodes macrocephala. 401~ 50010 ~16 Bupleurum chinense and roasted war. 25 1 ~ 4005 ~ 9 Angelica sinensis, Pericarpium Citri Tangerinae, Poria, Fructus Aurantii Immaturus, Fructus Aurantii Immaturus. 150 ~ 2503 ~ 7 Endothelium Corneum Gigeriae Galli, Fructus Crataegi, Rhizoma Dioscoreae, Rhizoma Pinelliae, Radix Aucklandiae and Radix Aconiti Lateralis Preparata. 101~149 3 ~ 5 safflower, peach kernel, dogwood, kudzuvine root, magnolia officinalis, zedoary, sparganium, turmeric, malt, medicated leaven and jujube. 3 1 ~ 100 1 ~ 3 Pericarpium Arecae, Raphanus sativus, Areca catechu, Polygonatum odoratum, Radix Rehmanniae, Radix Ophiopogonis, Zingiberis Rhizoma, Caulis Perillae, Herba Epimedii, Fructus Rosae Laevigatae, Fructus Toosendan, Radix Saposhnikoviae, Radix Platycodi, Orange, Cortex Cinnamomi, Os Draconis, Oyster, Fructus Perillae. From the above 50 herbs, Buzhong Yiqi decoction is still the most commonly used medicine for gastroptosis, which shows that Buzhong Yiqi decoction is still the main prescription for treating this disease. Drugs that warm yang and regulate qi also account for a considerable proportion. The clinical application of nourishing yin and moistening dryness is more, but the frequency of use is not high, which may be related to the fact that deficiency of stomach yin is less common in this disease than deficiency of spleen qi. It is worth noting that drugs for promoting blood circulation and removing blood stasis have attracted people's attention. Many articles reflect that Fang Yaozhong's application of drugs for promoting blood circulation and removing blood stasis on the basis of syndrome differentiation and treatment is often helpful to improve the curative effect.
7.6 The stomach point for acupuncture (2 inches below xiphoid process and 0.5 inch on the right side of abdominal midline) penetrates the drooping point (2.0 ~ 2.5 inches below umbilicus) and twists in one direction. After the needle tip is wound by tissue, it is lifted upward. The doctor presses the lower bound of the stomach with his left hand, and when the upper abdomen bulges and the patient feels that the upper abdomen is full, the needle is taken every other day 1 time. Or moxibustion at Baihui, Qihai, Zhongyu and Shu Wei, with moxa sticks smoked at each point for 10 ~ 15 minutes, every other day 1 time. [ 1]
7.6. 1 Acupoint selection: Main point: Juque, under xiphoid process 1 inch. Matching points: Chengman (right) and dovetail.
Operation: Generally, only the main points are selected. If the effect is not obvious, match points are used instead. Select one point at a time. Wool needles are made of 28 ~ 32 7 ~ 8 inch long millineedles. The patient lies flat, relaxes abdominal muscles and breathes evenly. Juque point needling method: the tip of the needle quickly enters the skin and goes straight to Zuo Shu point along the subcutaneous. Then, the handle holding the needle is slowly lifted, making an angle of 45 degrees with the skin, so that the operator feels that the needle tip is heavy and the patient feels that the navel and lower abdomen are lifted. Lift the needle slowly, it takes 20 minutes for the first time, and it can be shortened to 3 minutes later. Acupuncture under xiphoid process 1 inch: at an angle of 30 degrees with the skin, acupuncture at 0.5 inch on the left side of umbilicus. After the above needling sensation appears, the needling angle should be changed to 15 degree angle, without twisting, and slowly lift for 40 minutes. Needle shaking 10 ~ 15 times. -Shu Tian's left is a penetrating needle. Also use the above-mentioned vibration-boosting method, acupuncture Kyubi no Youko point to the lower left umbilicus with obvious tenderness point. After acupuncture, lie on your back for 2 ~ 3 hours, about 20 days 1 time, 3 times as a course of treatment.
Efficacy: * * 1047 cases were cured, 302 cases were cured, 23 cases were markedly effective, 428 cases were effective and 86 cases were ineffective, with a total effective rate of 9 1.8%.
7.6.2 Acupoint selection by electroacupuncture: The main acupoints are Zhongwan, Shang Wei, Wei Ti and Qihai. Matching points: Zusanli, Neiguan and Pishu.
Position of stomach-lifting point: 4 inches next to Zhongwan point.
Operation: Take 2 ~ 3 points at a time, and add matching points as appropriate. Qihai point 1 ~ 1.5 "is straight, and the upper abdomen, upper stomach and stomach lifting are all inclined downward at an angle of 45 degrees 1.5 ~ 2". Connect the intermittent electrotherapy machine, the negative electrode is connected to the middle cup, and the positive electrode is divided into five forks, which are respectively connected to the stomach, the upper stomach and the upper stomach and the air sea. Using density wave, the power is suitable for the patient's abdominal muscle contraction tolerance. Each time lasts * * * 20 ~ 30 minutes. Every day 1 time,1time is a course of treatment, and the interval of treatment is 3 ~ 7 days.
Efficacy: 25 1 case was cured, 75 cases were cured, 86 cases were markedly effective, 46 cases were effective and 44 cases were ineffective, with a total effective rate of 82.5%. Some cases also cooperate with acupoint injection.
7.6.3 Body acupuncture 7.6.3. 1: One selected point: main points: Jianli, Zhongyu, Shu Tian, Qihai and Shili. Matching points: Shangwan, Neiguan, Ermen, Gongsun, Pishu and Shu Wei.
Operation: take 1 ~ 2 points from the main point at a time, and match 2 ~ 3 points. Acupuncture abdominal acupoints in supine position. Jianli point should be directly pricked with a single needle until it gets angry. Shu Tian pricked Qihai with a 4-inch filiform needle at an angle of 15 degrees. From shallow to deep, three kinds of needling methods are adopted: the first needling method is about 5 o'clock, and the bird pecking technique is used until it becomes angry; Then acupuncture for about 8 minutes, using the same method to promote the strong feeling of acid swelling to spread to the upper and lower abdomen; Then three times to the required depth (generally to 1.2 ~ 1.5 inch), the manipulation is the same as before, the patient feels the feeling of acid swelling and contraction in the stomach, then twist it to the left or right for 3 ~ 4 times in the same direction, stop for half a minute, then twist it 1 time, and then take out the needle after the needle feels firm. Acupuncture points on the back, taking prone position, and inserting the needle in the direction of oblique intervertebral foramen 1 ~ 1.5 inch, and adopting the complementary method. Leave the needle for 30 minutes, directly stab the limbs, use the tonic method, and also leave the needle for 20 ~ 30 minutes. Every day or every other day 1 time. Lie flat 1 ~ 2 hours after treatment. 10 is a course of treatment, and the interval of treatment is 5 ~ 7 days.
Efficacy: 453 cases were treated with * * * *, of which 157 cases were cured, 149 cases were markedly effective, 126 cases were effective and 2/kloc-0 cases were ineffective, with a total effective rate of 95.4%.
7.6.3.2 Fang Er used stomach points (2 inches below xiphoid process, 0.5 inches beside the right side of belly line) to pierce the drooping points (2.0 ~ 2.5 inches below umbilicus) and twisted in one direction. After the needle tip is wound by tissue, it is lifted upward. The doctor presses the lower bound of the stomach with his left hand, and when the upper abdomen swells and the patient feels full, the needle is taken every other day 1 time. [ 1]
7.6.4 Acupoint application and acupoint selection: Baihui.
Surgery: The drugs were divided into two groups. Group A: castor seed 10g, Cimicifuga powder 2g; Group B: 9.8 grams of castor seeds and 0.2 grams of gallnut powder. Mashing castor seeds into mud, mixing with Cimicifuga powder or Galla Chinensis powder, and making into medicinal cake with a diameter of 2-3 cm and a thickness of 65438+/-0 cm. Shave off the hair of a large piece of medicine cake at Baihui point, paste any medicine cake, fix it with bandage, let the patient take a horizontal supine position, relax his belt, and scald the medicine cake with a salt bottle filled with hot water at about 80℃ three times a day, each time 10 ~ 30 minutes. Each medicine cake can be used continuously for 5 days. Treatment should be carried out 2 hours after meals. It is not suitable for people with heart disease, hypertension and hemoptysis and pregnant women.
Efficacy: 329 cases were treated with * * * *, of which 105 cases were cured, 98 cases were markedly effective, 96 cases were effective and 30 cases were ineffective, with a total effective rate of 90.88%.

7.7.2 The following methods in Formula 2 are effective for patients with gastroptosis such as stomachache, nausea, abdominal distension and belching [3]:
① The palms of both hands overlap, and the upper abdomen is pushed from Shenque to the left 100 times.
② Put the middle finger and the third finger together, and press the abdomen with the fingertips to rub the abdomen 1 min.
③ Press Shu Wei with both thumbs for 20 times.
④ Press the thumb on both sides of Zusanli 1 min.
7.7.3 Precautions: Massage techniques for patients with gastroptosis should be gentle, relaxed, light and skilled, and the intensity should be from light to heavy. At the same time, we should pay attention to the change of anatomical position to make the manual treatment accurate [2].
7.8 qigong therapy operation: ① internal strength: posture from supine position to lateral position to sitting position, depending on physical condition. Breathing should be based on natural and gentle breathing. On the basis of relaxation, C should pay attention to the navel, and people with abdominal distension can cooperate with reverse breathing, thin stool and levator ani breathing. ② External exercises: routine exercises or Tai Ji Chuan, 1 ~ 2 times a day. Abdominal distension and abdomen rubbing 100 ~ 300 times, backache and abdomen rubbing 100 ~ 300 times.
Efficacy: 252 cases were cured, 44 cases were cured, 99 cases were markedly effective, and 109 cases were effective, with a total effective rate of109%.
7.9 Medical record Wang, female, 44 years old. I felt sick in my stomach in 2000. After the treatment of traditional Chinese and western medicine, the effect is not obvious, and the symptoms are getting more and more serious. Loss of appetite, stomachache after meals, ineffective after taking about 150 dose of traditional Chinese medicine, emaciation, palpitation and insomnia.
