1. In case of any change in the registered items of the qualification certificate, an application shall be submitted to the corresponding issuing unit within 30 days according to the qualification grade;
2, enterprises in the province to change the relevant information should submit the following materials;
(1) enterprise qualification change information application form enterprise qualification change application form;
(2) A copy of the business license of the enterprise;
(3) Original qualification certificate (if the name of the enterprise is changed, the original qualification certificate shall be provided);
(4) Other certification materials related to qualification change.
(1) If the enterprise is restructured as a whole, the approval document of enterprise restructuring or the resolution of shareholders' meeting or workers' congress shall be provided;
(2) The person-in-charge of the unit and the technical person-in-charge shall provide a copy of the post-holding documents and their ID cards, professional titles, relevant qualification certificates and academic certificates;
3. Where the registered place of an enterprise is changed across provinces, the enterprise shall apply by changing the accepting unit in the province where it is located, and the accepting unit in the province where it is changed shall be responsible for handling the change procedures. In addition to the above materials, the following materials shall be provided:
(1) The original application form for enterprise qualification change information shall be approved and sealed by the accepting unit in the province where the original qualification certificate was changed;
(2) an application form for qualification;
(3) Copies of labor insurance certificates and labor contracts of professional and technical personnel listed in the qualification application form;
(4) Certificate of deregistration of the original enterprise in industrial and commercial registration before the change of enterprise qualification.
